Registrar and Academic Records Office -- Transcripts

General Information

We do not accept phone or e-mail requests for transcripts. Transcript requests, by law, must be signed by the person who attended the course.

Transcripts are only issued after receipt of a written request or DINFOS Form 306 (20) from the student. The following information is required in the request: name (maiden name), social security number, date of birth, home address, class(es) attended with date(s) of graduation or class number, and student's signature. Lastly, remember to give an address of the educational institution that you wish us to send an official transcript, and remember to sign your request.

Transcripts are released in accordance with the guidance provided in the Academic Record and Transcript Guide, and from the Accrediting Commission of the Council on Occupational Education.

To request transcripts by mail, send a letter including the following information:

Your full name and/or maiden name.

Your birth date and social security number. Your approximate date of attendance (and permanent number if known). The full name and address of where you would like your transcripts to be sent. A phone number or address where you can be reached for any questions. Signature.

What kind of transcript do I request?

Official Transcripts

Transcripts sent from the Defense Information School (DINFOS) to a college are con­sidered to be official. Most colleges and universities do not accept transcripts issued to the student.

No partial transcripts will be issued.

In accordance with the Family Educational Rights and Privacy Act of 1974, records may not be released to a third party without prior written authorization of the student with a notarized signature.

Unofficial Transcripts

Only official transcripts will be sent to universities/colleges/service education offices. Requester may ask for a file copy to be sent to their home or business address. A file copy is not an official copy.

All transcripts from The Defense Information School (DINFOS) are free.

All requests for transcripts will be processed in a timely manner. The process will take approximately two to four weeks, depending on the time of year. Please under­stand that we do not rush-process any request. In fairness to all, we fill transcript re­quests in the order we receive them. Please plan ahead.

Transcripts are sent first class U.S. mail. Please understand that we fill transcript re­quests in the order we receive them.

This form is available for downloading only. Since the requester must sign the form, it can be printed out, signed, then mailed or faxed.
